A Clinical Analysis fof the Appendicitis in Children



Abstract
This study is a clinical analysis of 269 cases of the appendicitis in children under the age of 15, who were operated at Department of Surgery, Hae-Sung General Hospital form August 1988 to May 1991.

@EN For better analysis, we divided our cases into three groups, that is perforated group. nonperforated group, and misdiagnosed group.

2) Overall perforation rate was 1.2 : 1. 14.9%. and perforation rate was the highest in the group under the age of 5(35%). Overall misdiagnosed rate was 21.9%.
3) The mean duration of symptom was 81.6 hours in perforated group, 40.8 hours in nonperforated group and 45.6 hours in misdiagnosed group. The longer duration of the symptom was. The higher rate of perforation and misdiagnosed group were.
4) The most common symptom and sign of acute appendicitis were right lower quadrant abdominal pain and tenderness. And diffuse abdominal tenderness, rigidity, palpable mass and decreased bowel sound of perforated group were more common in
perforated
group than the other groups.
5) The leukocytosis(over 10.000 mm) was found in 100% of perforated group, 77.1% of nonperforated group and 55.9% of misdiagnosed group. The mean WBC count was 19,300/§± in perforated group, 14.2000/§± in nonperforated group, and 11,500/§± in
misdiagnosed group.
The mean neutrophil segment count was 85.3% in perforated group. 17.0% in nonperforated group and 75.6% in misdiagnosed group. And it was statistically significant between perforated group and the other groups. And the more increased neutrohil
segment
count was, the higher rate of perforated group. And the rate of perforated group was significantly increased at above 86% of neutrophil segment count.
6) Intraoperative culture was positive in 92.8% of perforated group and 35.8% nonperforated group. E. coli was the common pathogen in the culture.
7) The postoperative complication was developed in 13 cases(32.5%) in perforated group and 4 cases(0.02%) in nonperforated group. Wound abscess was the most common complication(64.7%).
8) The mean duration of hospitalization was 13.8 days in perforated group, 6.9 days in nonperforated group and 7.0 days in misdiagnosed group.
